The forms provided below are intended only for new clinical patients. If you have been referred to our office by an attorney or the court, please contact us directly concerning the forms to be completed and the types of information we are requesting from you.

We appreciate your interest in completing paperwork prior to your appointment. You will save yourself time in the waiting room by downloading, printing, completing, and bringing them with you. Please find a private location to read and complete these forms. When you arrive with them at your first appointment you will be asked to attest to the fact that you alone completed the forms, or ask to describe how they were completed.
